What is Agomelatine?
Agomelatine is closely related to melatonin in structure. Agomelatine is a potent melatonin receptor agonist and serotonin 2C (5-HT2C) receptor antagonist, tested in animal models of depression.
Medical use
Major depressive disorder
In Europe, agomelatine is used to treat severe depressive episodes in adults. Ten placebo-controlled trials have been conducted to study the short-term effect of agomelatine on major depression. At the end of treatment, six out of ten short-term double-blind placebo-controlled studies demonstrated significant efficacy. Two trials were considered “failed” because the control product with established efficacy cannot be distinguished from the placebo. In all positive placebo-controlled studies, efficacy was also observed in patients with more severe depression. A study to prevent recurrence proved the maintenance of the efficacy of antidepressants. A meta-analysis found that agomelatine was as effective as standard antidepressants.
Use in special populations
Due to a lack of safety and effectiveness data, Europe does not recommend its use for children and adolescents under 18 years of age. However, a study reported in September 2020 showed that 25 mg of agomelatine per day is better than a placebo in young people aged 7-18 years. There are limited data on use in elderly people over 75 years of age with major depression.
Not recommended for use during pregnancy or breastfeeding.
Side effects
Common side effects include weight gain, fatigue, liver problems, nausea, headaches, and anxiety. Due to potential liver problems, continuous blood tests are recommended. It is not recommended for people with dementia or people over 75 years of age. There is preliminary evidence that it may have fewer side effects than some other antidepressants. It works by stimulating melatonin receptors and blocking serotonin receptors.
